What is a care plan?

GP Chronic Condition Management Plan is a Medicare initiative designed to support people with chronic medical conditions. If you’re experiencing physical health challenges such as musculoskeletal issues, your GP may refer you to an osteopath for treatment.

You are allocated 5 care plan session per calender year, which expires within 18 months of the referral. Once all 5 are used, standard fees will apply for any additional visits

Who Can Benefit from a Care Plan?

If you’re dealing with chronic pain or health conditions, a chronic condition management plan can provide significant relief and improve your quality of life. Common conditions that benefit from osteopathy include:

  • Musculoskeletal pain

  • Arthritis and joint pain

  • Chronic back pain

  • Neck and shoulder stiffness

  • Post-surgery recovery

  • Sports injuries

How Can Osteopathy Help?

Osteopathy is a holistic approach to managing musculoskeletal conditions and improving overall physical health. It focuses on diagnosing and treating conditions related to the muscles, bones, and joints through hands-on techniques such as massage, stretching, and joint manipulation.

Osteopathic treatment can help with:

  • Pain relief

  • Improved mobility

  • Reduced muscle stiffness

  • Faster recovery from injuries

To be able to book you a care plan appointment and provide a rebate for you we require a referral letter form from your GP.
